Two medical clinics in Lincoln City are consistently providing excellent patient-centered care – and their efforts were rewarded with 5-Star designations by the Oregon Health Authority.

Samaritan Lincoln City Medical Center and Samaritan Coastal Clinic join 37 other clinics in Oregon that are currently recognized as 5-Star clinics.

All Samaritan Health Services primary care clinics participate in the Patient-Centered Primary Care Home program, including clinics in Newport, Toledo and Waldport. The goal is for all Samaritan primary clinics to attain Tier 4- or 5-Star status in 2019 and 2020.

“This is not an easy designation to receive. It takes a concerted effort by all clinic staff to deliver exceptional and coordinated care,” said Kimberanne Silbernagel, clinics director at Samaritan North Lincoln Hospital. “I appreciate every team member at both clinic sites for their hard work, cooperation and dedication. They are true, caring professionals.”

The Oregon Health Authority oversees the PCPCH program. Its goal is to create clinic teams that emphasize transformative, whole-person, quality and evidence-based care. At its heart, this model of care fosters strong relationships with patients and their families; reduces costs; and improves care by catching problems early, focusing on prevention, wellness and managing chronic conditions.

“By collaborating in this manner, we hope to provide better care and better health to every patient and with lower costs,” Silbernagel explained. “In other words, we are helping patients navigate the health system and get the care they need in a safe and timely manner.”

To earn 5-Star designation, clinics submit a comprehensive application then OHA staff make a site visit and conduct a patient focus group to verify that several core attributes of patient-centered care are being followed, such as ease of scheduling timely appointments, ability to see the same clinician, and ease of receiving referrals for specialty care, to name a few of the four dozen standards.
